ParentsWeb: Magnus Health 2021-2022

Required Medical and Athletic Forms for the 2021-2022 school year are due no later than July 30, 2021.

Please contact your health care provider now, so they can advise you on completing forms and getting immunizations and physical exams updated. Please note: All questions about COVID-19 are optional and for informational purposes only. 

To access your student’s Magnus Health account:

  1. Log in to your ParentsWeb account. The District Code is JB-PA. Your username and password were chosen by you when you created the account. (If you forget your login, you can use the "Forgot Username / Password?" option.)

  2. Select “Student” in the main menu, then click “Medical”. Click on the direct link to Magnus Health.

  3. Enter the required health information and upload the required health forms within your Magnus Health account for each child attending Barrack no later than July 30, 2021

Timely submission of health information and other forms is vital so that preparations for students’ medical needs can be made prior to the first day of school on September 2. You will receive weekly reminders as long as there are outstanding items in your account. Please find below a list of resources that will be helpful regarding Magnus.

As a reminder, Barrack’s Health Office is entirely paperless. Please do not mail any medical forms. All medical information for Barrack students is stored in Magnus Health and must be uploaded by parents / guardians.

If you have difficulty scanning and uploading forms, you can follow these simple instructions for 

Here is a checklist of forms that you will be required to complete or upload to Magnus Health. Your account will only display forms that are required for your child. 

Required School Forms include:

  1. Vital Health Record -- completed by parent electronically

    • All Students

  2. Immunization Records -- provided by doctor and uploaded

    • All New Students

    • All 6th, 7th, 11th and 12th graders

      1. 7th graders require proof of two immunizations (Tdap and MCV)

      2. 12th graders require proof of MCV #2 

  3. School Physical -- to be downloaded and signed by doctor

    • All New Students

    • All students participating in sports

    • All 6th graders

    • All 11th graders: The Muss Physical form will be used for your student's required 11th grade physical for Barrack. If you plan to attend Muss then please also upload this form to AMHSI.

  4. Medication Authorization form -- to be downloaded and signed by doctor

    • To be completed for ALL students requiring daily medication at school or on overnight field trips.

  5. Dental Form -- to be downloaded and signed by dentist

    • All 7th graders

    • All incoming students new to Pennsylvania.

  6. When to Keep Your Child Home -- to be signed by parent electronically

    • All Students

  7. Textbook Authorization form -- to be signed by parent electronically

    • All Students

  8. 2021 Flu Vaccination -- Required for all students and due by October 15

    • This is a required form that must be completed and signed by a parent/guardian. *** If you plan to receive the 2021 flu vaccination at a future date, wait to answer until vaccination is completed. Only answer NO if a medical/religious exemption is required. A doctor’s note will be required for exemption from the flu vaccine.

  9. COVID-19 Vaccination Card (OPTIONAL)

    • We ask if your student has completed the COVID-19 immunization. If YES, we ask that you voluntarily upload a copy of their vaccination card. This will assist us if there is a need to quarantine students.

Sports Participation Forms: (Please complete, sports will take place dependent upon state, county and township guidelines regarding COVID-19)

  1. School Physical (same as above) for all students participating in Barrack sports program -- to be downloaded and signed by doctor

  2. Sudden Cardiac Arrest Warning Signs -- to be signed by parent electronically

  3. Parent/Athlete Concussion Information -- to be signed by parent electronically

  4. Treatment Consent -- to be signed by parent electronically

Additional Action Plans (optional, as needed)

  • Food Allergy Action Plan -- to be signed by doctor and uploaded

  • Diabetes Action Plan -- to be signed by doctor and uploaded

  • Asthma Action Plan -- to be signed by doctor and uploaded

  • Seizure Action Plan -- to be signed by doctor and uploaded
